See here for the two forms “Application for the admission” and “Concept of the doctoral thesis”, as well as below in the same page, the list of personal documents needed for the admission: https://www.meduniwien.ac.at/web/en/studies-further-education/application-admission/phd-and-doctoral-programmes/

Concerning the form “Concept of the doctoral thesis”: note that this form needs to be filled by you together your supervisor, as some planned project details need to be indicated and the supervisor needs to sign it, too.

All documents must be provided in English or German and may need an authentication. Note that if your documents were not issued in English or German originally you need to have them translated by a certified translator and then authenticated. See the detailed information on the translation and authentication of documents in the MedUni link mentioned above, under “Documents and Authentofication”. The kind of authentication always refers to the state in which the document was issued. So if you lived and studied in different countries you may need different authentications for the single documents.
